Informed consent is always a challenging aspect of research, particularly in the long term care arena. But you don’t have to re-invent the wheel when it comes to informed consent forms. Several useful examples, sample forms, and templates can be found on the Internet.

These include a VA System Sample Consent form, available at http://humansubjects.stanford.edu/medical/VASampCons.html. A sample form used by Stanford University researchers can be accessed at http://humansubjects.stanford.edu/medical/SUSampCons.html.

Many protocols are returned to researchers because of problems with consent forms. A copy of a three-part “Informed Consent Checklist” of elements that IRB panel members look for when they review consent forms can be downloaded from http://humansubjects.stanford.edu/medical/consentcheck.html
