As discussed in the April e-Newsletter, one of our goals at the LTC-QI Partnership is to help nursing homes engage in behavior change by linking the CPGs with helpful tools, education, and training.

Last month, we described how the LTC-QI Partnership staff provides participating nursing homes with: (1) training about the CPG; (2) CPG-based policies and procedures; and (3) data collection tools to self-audit their progress on key care processes.
