At AMDA and the LTC-QI Partnership, one of our goals is to help nursing homes engage in behavior change by linking the CPGs with helpful tools, education, and training.


For example, the LTC-QI Partnership staff provides participating nursing homes with:

  • Training about the CPG that includes strategies and tools to use when incorporating the CPG into their nursing home's care processes;
  • CPG-based policies and procedures (referenced in the box above) that enable nursing homes to create clear and consistent policies in order to ensure that all staff adhere to essential care processes; and
  • Data collection tools to help nursing homes to collect medical records data and examine their performance on process of care measures to: (1) see their strengths and weaknesses and (2) evaluate the impact of systems changes.
