As a reminder, there are three data collection periods (DCPs) in the LTC-QI Partnership:



  1. DCP #1 (Baseline) - Month 1
  2. DCP #2 (Follow-up) - Month 9
  3. DCP #3 (Sustainability) - Month 15

As the Pennsylvania homes finish their data collection (see box at right), other participating nursing homes continue to abstract and submit medical records data using our Web-based data collection tool. The tool allows nursing homes to collect and submit data using a confidential, user-friendly tool.

Audit & Feedback

Once they've submitted their data, nursing homes can immediately evaluate their performance by viewing their process measure scores and comparing their scores to their peers' aggregate scores. Using audit & feedback makes data collection a valuable part of the intervention.
