 Vol XXVI, Feb `07 With data collection nearly complete, our subcontractors at Quality Partners are set to begin data analyses to evaluate the project’s impact. As a reminder, we will be evaluating the publicly reported outcome measures and also the following process measures:
Pain:
- Change in medication regimen
- Complete pain assessment
- Non-drug therapy as part of plan of care
- Notifying MD, NP or PA of daily, moderate to severe pain
- Pain diagnosis
- Treating daily pain with regularly scheduled pain medications
- Treating moderate to severe pain with WHO II or III pain medications
- Use of pain intensity scale
Pressure Ulcers:
- Complete wound description
- Pressure-reducing support surface on bed
- Pressure-reducing support surface on chair
- Repositioning residents every two hours during the day
- Timely addressing of pressure ulcer risk factors in plan of care
- Timely risk assessment for residents with pressure ulcer(s)
- Timely risk assessment for recently admitted residents
- Unidentified / missed pressure ulcers in high-risk residents
- Weekly complete skin examination
- Weekly update on wound healing
