With data collection nearly complete, our subcontractors at Quality Partners are set to begin data analyses to evaluate the project’s impact. As a reminder, we will be evaluating the publicly reported outcome measures and also the following process measures:
Pain:
  • Change in medication regimen
  • Complete pain assessment
  • Non-drug therapy as part of plan of care
  • Notifying MD, NP or PA of daily, moderate to severe pain
  • Pain diagnosis
  • Treating daily pain with regularly scheduled pain medications
  • Treating moderate to severe pain with WHO II or III pain medications
  • Use of pain intensity scale
Pressure Ulcers:
  • Complete wound description
  • Pressure-reducing support surface on bed
  • Pressure-reducing support surface on chair
  • Repositioning residents every two hours during the day
  • Timely addressing of pressure ulcer risk factors in plan of care
  • Timely risk assessment for residents with pressure ulcer(s)
  • Timely risk assessment for recently admitted residents
  • Unidentified / missed pressure ulcers in high-risk residents
  • Weekly complete skin examination
  • Weekly update on wound healing
