Eligibility
The application deadline is November 20, 2009. Applicants must complete an online application and attach a Curriculum Vitae (CV) and a Letter of Interest (no more than 250 words, describing your interest in long term care practice and why you feel you will benefit from participating in this program). A Letter of Support from the applicants program director or another AMDA member is also required. To Apply All Futures applications must be submitted online where attending documents can be uploaded in electronic form. Applications received in our office other than through our web site will not be reviewed. The online application submission process is only accessible by registered users of our site. Please see My Area for more information on registration. You may log in and gain access to the online application once you are registered. You may then complete and submit the online AMDA Foundation Futures Program Application from the Programs menu once you're logged in. Method of Selection The AMDA Foundation Futures Selection Committee ranks the applicants according to their Letter of Interest, CV and Letter of Support and makes selections based on the highest scores. Priority is given to poster presenters at the Long Term Medicine 2010 (formerly the AMDA Symposium) in Long Beach. Please do not make any travel or hotel arrangements until you are notified of your acceptance.
