Congratulations to Practice Change Fellows |

Alice Bonner, PhD, RN, a 2007 AMDA Foundation/Pfizer QI Award winner was among ten persons named as Practice Change Fellows for 2009. Bonner, of the Massachusetts Senior Care Foundation was named for her project, “A Statewide Program to Improve Care Transitions and Reduce Avoidable Rehospitalizations from Skilled Nursing Facilities.”
AMDA member Randi Berkowitz, MD, CMD of Hebrew Senior Life was
also named for his project, “Rehospitalization Reduction Program on a
Geriatric Skilled Nursing Unit.” The Practice Change Fellows program is
a two year opportunity for nurses, physicians, and social workers to
develop leadership skills and content expertise with a $90,000 award in
order to positively influence care for older adults. Fellows complete a
project aimed at implementing a new geriatric program or service line
integrated within their organization, allowing them to remain at their
full time job throughout the two year program. The program is funded by
the Atlantic Philanthropies and the John A. Hartford Foundation.
