Columbia, MD — In an effort to promote quality care and quality improvement (QI) programs and models that have produced positive results in long term care facilities, the AMDA Foundation launched its first quality improvement (QI) awards program over 10 years ago. Now the Foundation introduces a new honor, the Quality Improvement & Health Outcome Awards that continues this strong tradition of focusing on QI and best practices in quality care. This award was designed to honor innovative programs implemented by nursing homes to improve resident care and quality of life.

Three awards will be presented at LTC Medicine-2012, March 8-11, 2012, in San Antonio, Texas, where honorees will present their findings to medical directors and physicians from across the country and internationally. Further, award winners will be spotlighted in AMDA publications and join our esteemed list of QI Program Award recipients on the Foundation website where the LTC community can review their QI program and learn from their successes...

“As the professional association of medical directors and physicians practicing across the long-term care continuum, AMDA constantly seeks for new ways to inspire best practices in the industry,” said Jonathan Musher, MD, CMD, Past Chair, AMDA Foundation.

The types of programs that might be selected for these awards include:

  • Initiatives on patient-centered care
  • Patient safety initiatives – reducing falls & medication errors
  • Reduction of avoidable ER visits and acute hospitalizations
  • Intergenerational activities
  • Improved consistency of staffing
  • Improved comprehensive advanced care planning
  • Improved palliative care programs.

The programs selected for awards must be generated and funded by the nursing home. Programs need to have demonstrated measurable outcomes and objectives, as well as sustainability and an ability to be replicated in other facilities. At least one award will be designated specifically for improved advanced care planning and/or palliative care.

Past Foundation QI award winner Tobie Olsan, MD, said, “This award extends our Consortium’s impact by pooling our knowledge and resources across homes to improve transitional care in the entire region. Ultimately, this work will make a difference in residents’ lives by improving the continuity and coordination of care.”

The application deadline is November 21, 2011; winners will be notified no later than January 15, 2012. A group of experts appointed by the AMDA Foundation will select award recipients. All nursing home facilities are eligible for these awards, and facilities may nominate themselves.
